Dear Parent /Guardian,


It is hard to believe that Thanksgiving is almost upon us. Teacher conferences will be held on Monday and Tuesday next week. Our teachers are available in the evening Monday.  Please contact the teacher directly to schedule a conference. If you prefer a meeting with all of your student’s teachers, please contact your counselor so that they can facilitate this. Students will not be in session next week, except for those who attend the NMTCC.


Next week the NMTCC is holding classes on Monday and  Nov 25 and Tuesday Nov 26. Transportation is provided for both AM and PM technical students to attend their session at NMTCC.  Since there is not school at PVHS a bus will pick up our NMTCC students at their respective bus stops.  Please check with your tech bus driver to find out more information.


Senior graduation project paperwork is due December 5th.   

  • Time Log (documenting 24 hours on your project)

  • Mock Interview Form (with signatures)

  • 3 page paper (4 copies, please)

Perkiomen Valley School District is proud to support families in need this holiday season through the fourth annual PV Cares Holiday Drive, which will be held Nov. 18 through Dec. 11 at each school.


All items are being collected to support the Daily Bread Food Pantry and the PV Power Packs program, both of which are serving PV families during the holiday season. Donations are designated by grade level:


Kindergarten/Grade 1

Toys for newborns through toddlers

Grade 2/3

Toys for girls (Polly Pockets, Legos, Hello Kitty, LOL Surprise!, PikMi Pops, My Little Pony, Sequin Pets (Unicorn) items)

Grade 4/5

Toys for boys (Legos, Hot Wheels City sets, Star Wars items)

Grade 6/7

Toys for boys or girls (board games, Paw Patrol items, Nerf toys, V-Tech toys)

Grade 8/9

Gifts for girl teens (movie tickets/ultimate packs, make-up/bath kits, nail polish kits, hoodies, lounge pants)

Grade 10/11

Gifts for boy teens (movie tickets/ultimate packs, hoodies, lounge pants, cologne)

Grade 12

Gift cards to Target or Wal-Mart (smaller denominations preferred)


Donations can be dropped off at any school until Dec. 11 and placed in the blue barrels. We thank you in advance for your generosity this holiday season.


If you are a family who is need of assistance this holiday season, bring this flyer to the Daily Bread Food Pantry or say you are with the “PV Cares” program. Gifts may be picked up Dec. 3-21 from 9 to 11 a.m. on Wednesdays and Saturdays, or from 6 to 8 p.m. on Thursdays. Daily Bread is located at 3938 Ridge Pike, Collegeville, PA 19426.

Physical Exam for Grade 11

2019-2020


Dear Parent /Guardian:


PA state law requires a medical examination for all students in the eleventh grade. It is recommended that the exam be performed by your family physician since he/she can best evaluate your child’s health and update immunizations. An exam completed on or after July 1, 2018 (up to one year prior to the student’s entry into eleventh grade) will suffice. Please return the completed form to your child’s school nurse as soon as possible. It is a good idea to make a copy for your records or scan via email.  


While visiting your doctor, please get written permission for any prescription or over-the-counter medication that may be needed during the 2019-2020 school year. Medication permission forms can be found on the school website or contact your school nurse for a copy.    


Please note:  The PA Department of Health immunization recommendations for this age group include a 2nd dose of meningococcal conjugate vaccine (MCV).  This is also a requirement for entry into 12th grade.

Please submit all requests for educational trips at least five (5) days prior to the trip. The form must be filled out and signed by the parent and contain the signatures of the student’s teachers prior to being turned in to Mrs. Berry, our attendance secretary.  The form can be found on our website under FORMS in the parent tab. Students who are going on college visits must also turn in an educational trip form and if proof of the visit is supplied the trip will not be counted as an absence.
